• Health Conditions
  I confirm that the health and allergy information I have provided is accurate and complete. I understand it is my responsibility to inform Salt & Stone Massage Bar of any changes to my health or medication before any massage.

• Allergy & Sensitivity Information
  I have disclosed any known allergies or sensitivities, including (but not limited to) massage oils, essential oils, fragrances, nuts, herbs, or any plant-based or synthetic ingredients (e.g., coconut oil, almond oil, jojoba oil). I understand this information is essential for the safe and effective delivery of massage services.

• Massage Risks
  I understand that massage therapy and hydro treatments may involve certain physical risks. While every precaution is taken to ensure comfort and safety, I accept that individual responses to treatment vary and that some discomfort or temporary aftereffects may occur.

• Client Responsibility
  I take full responsibility for notifying my therapist immediately if I experience any pain, discomfort, or adverse reaction during my massage. I understand that the therapist may modify or discontinue the session in response.

• Consent to Treatment
  I give full consent to receive massage or hydro treatments by a qualified therapist, delivered within their professional scope of practice and training.

• Confidentiality
  All personal, medical, and health information I provide will be handled with strict confidentiality, in accordance with South African privacy and data protection laws.

• Liability
  Salt & Stone Massage Bar, its owners, employees, and therapists are not liable for any injury, loss, or damage resulting from services provided, including in cases of gross negligence. Treatments are provided for therapeutic and wellness purposes only and are not a substitute for medical care.

Client Acknowledgement
By signing the Salt & Stone client intake form (welcome card), I acknowledge that I have read, understood, and accept the above terms, and I release Salt & Stone Massage Bar and its practitioners from any claims or liabilities arising from my massage.
